Sunrise Therapy Services is looking for an Occupational Therapist to join our team providing home based services to children and families in the Arizona Early Intervention Program (Birth to Three). West Valley Area

Essential Job Functions

As a member of the multidisciplinary team, the position includes providing evaluations, participating in child and family assessments, and providing ongoing therapy to infants and toddlers with developmental delays or disabilities. Services are provided in natural environments including, but not limited to, the child’s home, child care centers, parks and other community settings. The primary focus is to assist parents/caregivers in supporting their child’s learning and development within their everyday routines and activities.

Minimum Qualifications

  • A degree in Occupational Therapy.
  • Licensed as an Occupational Therapist in the State of Arizona ·
  • Pediatric experience is preferred

Job:  Physical Therapist

The Speech and Language Pathologist will participate in weekly core team meetings and Individualized Family Service Plan meetings, including reviews and transition planning meetings. The interested applicant must be open to a flexible schedule, and be willing to travel to provide services in natural environments in and around the west valley cities.

Minimum Qualifications

  • A Master’s degree in Speech and Hearing Sciences for Speech-Language Pathologist
  • Certificate of Clinical Competence from American Speech/Language Hearing Association
  • Licensed as a Speech Therapist in the state of Arizona
  • Pediatric experience with feeding, is preferred but not required
  • Candidates must pass background clearance
  • Bilingual English/Spanish is desired but not required

Job:  Licensed Social Worker

Because our programs continue to grow, we are currently looking for a Licensed Social Worker

Essential Job Functions

As a member of a multidisciplinary team, the Social worker may conduct initial evaluations to determine AzEIP eligibility. The candidate will participate in the child and family assessment process and in the development and implementation of the Individualized Family Service Plan (IFSP). The social worker must be available to provide social work services directly with the family, in a coaching capacity with another team member, or in a coaching capacity with the entire team.  The Social Worker facilitates family support and services through flexible, individualized and responsive interactions with the family and other team members. It requires a flexible schedule, with some evenings and/or weekends, if needed to support the family. The social worker is required to attend team meetings, as requested.  Travel to include home and community based visits within designated zip codes in Phoenix,

Position would be part-time to start.

Minimum Qualifications

  • A Bachelor’s or Master’s degree in Social Work.
  • Meet all prescribed professional requirements and rules of professional conduct as a Social Worker
  • Must have a valid AZ license (LBSW or LMSW)
  • Experience working with infants and toddlers and their families
  • Completion of the Arizona Early Intervention Program’s Standards of Practice is desired, but not required (if not completed, may be completed within the first three years of employment with Sunrise Therapy).
  • Candidates must pass background clearance.
  • Bilingual English/Spanish is a plus.
